Abstract

Background: Chronic ankle instability (CAI) significantly impairs neuromuscular control and dynamic balance in soccer athletes, contributing to recurrent injury and reduced athletic performance. Identification of dynamic balance deficits through the Y-Balance Test (YBT) enables targeted physiotherapy interventions.

Objective: To identify the dynamic balance profile of soccer athletes with CAI using the Y-Balance Test.

Methods: A descriptive quantitative study with a cross-sectional design was conducted on 30 active soccer athletes with confirmed CAI (CAIT ≤ 27) at AT Farmasi FC Surakarta in 2026, selected using consecutive purposive sampling. Dynamic balance was measured using the YBT in three reach directions—anterior (ANT), posterolateral (PL), and posteromedial (PM)—for both lower extremities. Composite scores were calculated by normalizing reach distances to limb length. Descriptive statistics and Shapiro-Wilk normality testing were performed using IBM SPSS Statistics v.26.

Results: Mean composite scores were 87.85 ± 10.63% (left) and 87.35 ± 10.88% (right), both below the commonly reported 89% reference threshold associated with increased lower-extremity injury risk. Sixteen subjects (53.3%) and 19 subjects (63.3%) scored below the threshold on the left and right limbs, respectively. Mean anterior reach asymmetry was 5.43 ± 5.55 cm, with 17 subjects (56.7%) exceeding the 4 cm clinical risk threshold.

Conclusion: Soccer athletes with chronic ankle instability predominantly demonstrated dynamic balance profiles below the commonly reported reference threshold, accompanied by a high prevalence of inter-limb asymmetry. These findings support the use of the Y-Balance Test as one component of functional screening in sports physiotherapy.
